Ritualist Program Record and Enrollment Form

Use this form to enroll in the Indiana Grand Lodge Ritualist Program.  Enrollment constitutes completing this form and sending it, along with a check for $10.00 made out to Indiana Grand Lodge, to the Clearinghouse.  Your points will be recorded and the appropriate pin will be forwarded to Lodge Secretary.
